Diabetes Resource Center

The Diabetes Resource Center at Lafayette General Medical Center is designed to help people with diabetes take control of their lives instead of letting the disease control them.  Anyone with diabetes, regardless of how long they have had it, can benefit from the services offered in our program.

Diabetes Self-Management Program
An individualized approach to living well with diabetes can help you achieve better management of the disease and more independence.  Patients can opt for four group sessions, over a one month period, or for individual instruction.  

The program covers:  
  • how to cope with diabetes
  • meal planning and nutrition
  • exercise and activity
  • taking medications properly, including insulin injections
  • blood sugar monitoring
  • prevention, detection and treatment of acute and chronic complications
  • foot, skin and dental care
  • community resources for care
Most importantly, patients develop a program that is right for them.  Group settings allow participants to learn from each other, while individualized consultations examine your specific needs.  

Other unique aspects of the LGMC program:
  • regular communication with your primary physician
  • encourages family members’ participation
  • flexible scheduling for counseling sessions
  • telephone follow-up program
Team Care
With the resources of a full-service hospital, our multidisciplinary team includes a nurse, dietitian, pharmacist, social worker, skin care nurse and exercise physiologist.  This team is critical to managing other symptoms of diabetes, such as cardiovascular disease. Only Lafayette General can offer the resources of a Top 100 Heart Hospital with a nationally accredited, self-management program.

Make An Appointment
All patients seen at the Diabetes Resource Center must be referred by a physician.  Classes are held at various times (day and evening classes are available).  Call 289-7329 to get started; we also welcome all other questions about diabetes, or to point you in the right direction to get started.  Medicare and most insurance plans are accepted. Check with a representative from Diabetes Resource to confirm the current status of Medicaid reimbursement.

Hours are:  Monday through Friday
from 8 a.m. to 4:30 p.m.
